Appeal for Reconsideration
-- (Completed by the Parent or the Student) This form should be submitted when making an appeal for reconsideration of special circumstances (e.g., loss of or reduced employment or excessive medical/dental expenses)
Education Abroad Expense Form
-- (Complete by the Student) Use this document as a fax cover sheet for the documents associated with the costs of your study abroad program.
Graduate Student Financial Aid Change Form
-- (Completed by the Student) This form should be submitted when reporting and/or requesting a change to a graduate student's financial aid award. This form should not be used by Medical, Law, or Graduate Business students.
Parent Non-Tax Filer Form
-- (Completed by a Parent of a Dependent Student) This form should be submitted when the parent(s) was not required to complete a federal tax return and/or to report untaxed income earned.
Retirement Plan Rollover Form
-- (Completed by the Student and the Parent) This form should be submitted to indicate a rollover from one qualified retirement plan to another, especially when the student's file has been selected for verification. In some cases, a complete copy of the relevant federal tax return for the year in question is required, so please see the form completion instructions for more information.
Student Non-Tax Filer Form
-- (Completed by the Student and/or Student's Spouse) This form should be submitted when the student and/or student's spouse was not required to complete a federal tax return and/or to report untaxed income earned.
Substitute Parent W-2 Form
-- (Completed by the Parent of a Dependent Student) This form should be submitted if the parent did not or will not receive a W-2 form for the 2016 tax year.
Substitute Student W-2 Form
-- (Completed by the Student) This form should be submitted if the student did not or will not receive a W-2 form for the 2016 tax year.
Forms Requested by Student Financial Services
The forms linked below are typically completed only at the request of Student Financial Services, usually by means of a To Do List item being placed on the student's SIS Student Center. Do not complete and submit any of the forms below unless specifically instructed to do so by Student Financial Services, as doing so will likely delay completion of your financial aid award.
Child Support Paid Form
-- (Competed by the Student and the Payer) This form is used to confirm the amount of child support paid by a parent or step-parent in 2015. For independent students, a parent may be the student or if married, the student’s spouse.
College Enrollment Verification Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the college enrollment of the student's family members.
Family Income and Expense Form
-- (Complete by the Parent of a Dependent Student or by an Independent Student) Detailed list of monthly income and expenses for 2016.
Federal Benefits Received Form
-- (Completed by the Student and the Parent) Generally, the Federal Benefits Received Form is requested because you reported that you or someone in your household received one or more of the following federal programs: Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP), Free or Reduced Price Lunch, Temporary Assistance for Needy Families (TANF), and Special Supplemental Nutrition Program for Women, Infants and Children (WIC), at any time during the 2016 or 2017 calendar year on the FAFSA.
High School Completion Status Form -- (Completed by the Student) This form is requested to confirm that you have a high school diploma or recognized equivalent. More than likely, you are being requested to submit this form because you indicated on the FAFSA that your high school completion status was 'home schooled' or 'none of the above'.
Household Size Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the number of dependents within a household. Generally, the Household Size Form is requested when the number reported in the household size and/or number in college on the (FAFSA and the CSS/PROFILE. This form may also be requested in order to confirm the identity of the family members reported in the household size and/or number in college on an application.
Household Support Form -- (Completed by a Dependent Student and the Parent or by an Independent Student) This form will be used to determine the contribution that each person makes to the household.
Independent Document of Support Form -- (Completed by an Independent Student) This form is used to document the financial support given to additional members of your household (e.g. child).
Non-Custodial Parent Form
-- (Completed by a Dependent Student's Non-Custodial Parent) This form should be submitted when providing information on a student's non-custodial parent.
Parent Marital Status and Custodial Parent Form
-- (Completed by a Dependent Student and Custodial Parent) Generally, this form is requested to confirm the marital status of the student’s parent(s), and in the case of divorce or separation, to determine the custodial parent for financial aid purposes.
Preparatory Coursework Certification Form -- (Completed by the Student) The Department of Education requires that Student Financial Services verify that students participating in preparatory coursework have not previously participated in preparatory coursework using Federal financial aid and that the student’s coursework is necessary for admission to a Post Baccalaureate program of study.
Trust Information Form
-- Requested when it appears that you and/or your parent(s) have one or more trusts. To proceed with determining your financial aid eligibility you must submit documentation of the trusts. Please review the Trust Information Form for information about what you are required to submit.
