Sign your 1040 tax forms in the signature box on page 2 and include all schedules. Tax returns that are missing a signature will be rejected. Submissions not including tax schedules may result in a delay in processing your financial aid.
Upload .pdf versions of your completed form through your SIS To Do List. Documents uploaded through the Student Information System are encrypted in transfer and are kept in a secure system vetted by the University’s Information Security Office. Submitted documents can only be accessed via a Virtual Private Network (VPN) by authorized personnel of Student Financial Services and the University. The information is used only for the purpose of needs analysis for financial aid eligibility.

Generally, the Federal Benefits Received Form is requested because you reported that you or someone in your household received one or more of the following federal programs: Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP), Free or Reduced Price Lunch, Temporary Assistance for Needy Families (TANF), and Special Supplemental Nutrition Program for Women, Infants and Children (WIC), at any time during the 2023 or 2024 calendar year on the FAFSA.
Completed by the student. This form is used to confirm that a student has a high school diploma or recognized equivalent.

Completed by an independent student. This form is used to document the financial support given to additional members of your household (e.g. child).
Completed by the student. Use this form to notify SFS of your intent to appeal for reinstatement of financial aid after suspension for failure to maintain Satisfactory Academic Progress. Someone from SFS will then contact you to discuss the requirements for an appeal.
Completed by the student. This form should be submitted when requesting that the parent who is not the parent of record on the FAFSA be permitted to waive the requirement to complete the CSS Profile.
Completed by a dependent student and custodial parent. Generally, this form is requested to confirm the marital status of the student’s parent(s), and in the case of divorce or separation, to determine the custodial parent for financial aid purposes.
