Typing your name or using an electronic image of your signature will result in SFS rejecting the form and requiring resubmission on your part.
Upload .pdf versions of your completed form through your SIS To Do List. Documents uploaded through the Student Information System are encrypted in transfer and are kept in a secure system vetted by the University’s Information Security Office. Submitted documents can only be accessed via VPN by authorized personnel of Student Financial Services and the University. The information is used only for the purpose of needs analysis for financial aid eligibility and is not shared.

Completed by the student. In the event a parent is declined for a Parent PLUS loan, this form is used to request to have the amount of the Unsubsidized loan increased accordingly. The student must have a FAFSA on file for the year.
Completed by the parent or student. This form should be submitted when making an appeal for reconsideration of special circumstances (e.g., loss of or reduced employment or excessive medical/dental expenses). Appeals for returning students will generally be considered after July 1, 2023.
Completed by the parent or student. This form should be submitted to inform Student Financial Services that an error has been made on the FAFSA and/or the CSS Profile that will have an impact on a student's financial aid. Completed by the student. Use this form to notify SFS of your intent to appeal for reinstatement of financial aid after suspension for failure to maintain Satisfactory Academic Progress. Someone from SFS will then contact you to discuss the requirements for an appeal.

The Department of Education requires that Student Financial Services verify that students participating in preparatory coursework have not previously participated in preparatory coursework using Federal financial aid and that the student’s coursework is necessary for admission to a Post Baccalaureate program of study.
Complete and submit this form if you will have financial aid and are generally expected to be a full-time student but have been approved by your school for less than full-time enrollment.
