2024-2025 Academic Year Forms

Please choose the correct form(s) for your needs and follow the instructions for completion and submission. You must sign all forms by hand (except for our DocuSign forms, which you will sign electronically). Typing your name or using an electronic image of your signature will result in SFS rejecting the form and requiring resubmission on your part. 

Sign your 1040 tax forms in the signature box on page 2 and include all schedules. Tax returns that are missing a signature will be rejected. Submissions not including tax schedules may result in a delay in processing your financial aid.

Upload .pdf versions of your completed form through your SIS To Do List. Documents uploaded through the Student Information System are encrypted in transfer and are kept in a secure system vetted by the University’s Information Security Office. Submitted documents can only be accessed via a Virtual Private Network (VPN) by authorized personnel of Student Financial Services and the University. The information is used only for the purpose of needs analysis for financial aid eligibility.

Additional Unsubsidized Loan Application Form

Completed by the student. In the event a parent is declined for a Parent PLUS loan, this form is used to request to have the amount of the Unsubsidized loan increased accordingly. The student must have a FAFSA on file for the year. 

This form will be available in early summer 2024.

Appeal for Reconsideration Form

Completed by the parent or student. This form should be submitted when making an appeal for reconsideration of special circumstances (e.g., loss of or reduced employment or excessive medical/dental expenses). Please contact our office by email at [email protected] to start a conversation about a possible appeal. See our Appeal for Reconsideration page for more information. Appeals for returning students will generally be considered after July 1, 2024. 

This form will be available in spring 2024.

Application Error Form

Completed by the parent or student. This form should be submitted to inform Student Financial Services that an error has been made on the FAFSA and/or the CSS Profile that will have an impact on a student's financial aid. To obtain a copy of the form or for more information on the submission process, please contact Student Financial Services directly at (434) 982-6000 or [email protected]. If you choose to write us rather than call us, please include the 7-digit reference number from your financial aid offer letter.

Asset Confirmation Request Form for Dependent Students

Completed by the parent for the dependent student. This form should be submitted when verifying Asset Information.

Asset Confirmation Request Form for Independent Students

Completed by the independent student. This form should be submitted when verifying Asset Information.

Consent to Obtain Credit Check

Completed by the parent. This form is used to grant consent to the University to obtain a credit report to determine eligibility for a Direct Parent PLUS loan.

This form will be available in early summer 2024.

Direct Parent PLUS Loan Application Form

Completed by the parent. This form must be submitted when applying for a PLUS loan for Undergraduate Students. PLEASE NOTE: In addition to this form, the parent must complete an application and Master Promissory Note at StudentAid.gov.

This form will be available in early summer 2024.

Education Abroad Expense Form

Completed by the student. Use this document as a cover sheet for the documents associated with the costs of your study abroad program. Complete this form for summer 2025 study abroad.

If you are studying abroad in the fall 2025 semester or for a later term, do not complete this form. Instead, navigate to the forms for the 2025-2026 academic year and find and complete the Education Abroad Expense Form for that year instead.

Family Income and Expense Form

Completed by the parent of a dependent student or by an independent student. Detailed list of monthly income and expenses for 2022.

Family Size Form for Dependent Students

Completed by dependent students. This form should be submitted when verifying the number of dependents within a family. Generally, the Family Size Form is requested when the number reported in the family size and/or number in college vary on the FAFSA and the CSS Profile. This form may also be requested in order to confirm the identity of the family members reported in the family size and/or number in college on an application.

Family Size Form for Independent Students

Completed by independent students. This form should be submitted when verifying the number of dependents within a family. Generally, the Family Size Form is requested when the number reported in the family size and/or number in college vary on the FAFSA and the CSS Profile. This form may also be requested in order to confirm the identity of the family members reported in the family size and/or number in college on an application.

Federal Benefits Received Form

Completed by the student and the parent. Generally, the Federal Benefits Received Form is requested because you reported that you or someone in your household received one or more of the following federal programs: Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP), Free or Reduced Price Lunch, Temporary Assistance for Needy Families (TANF), and Special Supplemental Nutrition Program for Women, Infants and Children (WIC), at any time during the 2022 or 2023 calendar year on the FAFSA.

Financial Aid Application Cancellation Request Form

Completed by the student, and the parent, if the student is a dependent student, who has completed the CSS Profile online.

Graduate Student Financial Aid Change Form

Completed by the student. This form should be submitted when reporting and/or requesting a change to a graduate student's financial aid offer. This form should not be used by Medical, Law, or Graduate Business students.

Health Insurance Expense Request Form

This form will be available after August 1, 2024.

High School Completion Status Form

Completed by the student. This form is used to confirm that a student has a high school diploma or recognized equivalent.

Identity and Statement of Educational Purpose Form

Completed by the student. This form is used to confirm a student's identity and certify that any federal student assistance received will only be used for educational purposes and to pay the cost of attending the University during the 2024-2025 academic year. 

Independent Document of Support Form

Completed by an independent student. This form is used to document the financial support given to additional members of your household (e.g. child).

Intent to Appeal for Satisfactory Academic Progress Form

Completed by the student. Use this form to notify SFS of your intent to appeal for reinstatement of financial aid after suspension for failure to maintain Satisfactory Academic Progress. Someone from SFS will then contact you to discuss the requirements for an appeal.

This form will be available in early summer 2024.

Noncustodial Profile Waiver Request Form

Completed by the student. This form should be submitted when requesting that the parent who is not the parent of record on the FAFSA be permitted to waive the requirement to complete the CSS Profile. Visit this page for more information on what may be grounds for a successful waiver request.

One-Time Computer Expense Request Form

Completed by dependent or independent students. This form should be submitted when requesting an increase to the Cost of Attendance for the purchase of a computer.

This form will be available in early summer 2024.

Outside Scholarships Self-Reporting Form

Completed by the Student or the Parent. Use this form to report outside scholarships or other non-UVA forms of funding.

This form will be available in early summer 2024.

Parent Marital Status and Custodial Parent Form

Completed by a dependent student and custodial parent. Generally, this form is requested to confirm the marital status of the student’s parent(s), and in the case of divorce or separation, to determine the custodial parent for financial aid purposes.

Parent Non-Tax Filer Form

Completed by a parent of a dependent student. Submit this form when at least one parent was not required to complete a federal tax return and/or to report untaxed income earned.

Preparatory Coursework Certification Form

Completed by the student. The Department of Education requires that Student Financial Services verify that students participating in preparatory coursework have not previously participated in preparatory coursework using Federal financial aid and that the student’s coursework is necessary for admission to a Post Baccalaureate program of study.

Reduced Course Load Notification Form

Complete and submit this form if you will have financial aid and are generally expected to be a full-time student but have been approved by your school for less than full-time enrollment.

This form will be available in late spring for the fall 2024 term.

Retirement Plan Rollover Form

Completed by the student and parent. This form is used to indicate whether a qualified retirement account was or was not rolled over into another qualified plan.

Student Non-Tax Filer Form

Completed by the student and/or student's spouse. This form should be submitted when the student and/or student's spouse was not required to complete a federal tax return and/or to report untaxed income earned.

TEACH Grant Request Form

Completed by the student. Use this form to apply for a TEACH Grant.

Trust Information Form

Requested when it appears that you and/or your parent(s) have one or more trusts. To proceed with determining your financial aid eligibility you must submit documentation of the trusts. Please review the Trust Information Form for information about what you are required to submit.

Undergraduate Student Financial Aid Change Form: Decrease

Completed by the student. This form should be submitted when reporting and/or requesting a decrease that will affect a student's financial aid offer.

This form will be available in early summer 2024.

Undergraduate Student Financial Aid Change Form: Increase

Completed by the student. This form should be submitted when reporting and/or requesting an increase that will affect a student's financial aid offer.

This form will be available in early summer 2024.

Untaxed/Unreported Income Confirmation Form

Completed by the student and parent. This form is used to confirm untaxed income received during a calendar year.
