Typing your name or using an electronic image of your signature will result in SFS rejecting the form and requiring resubmission on your part.

Additional Unsubsidized Loan Application Form -- (Completed by the Student) In the event a parent is declined for a Parent PLUS loan, this form is used to request to have the amount of the Unsubsidized loan increased accordingly. The student must have a FAFSA on file for the year.
Application Error Form -- (Completed by the Parent or the Student) This form should be submitted to inform Student Financial Services that an error has been made on the FAFSA and/or the CSS/PROFILE that will have an impact on a student's financial aid. To obtain a copy of the form or for more information on the submission process, please contact Student Financial Services directly at (434) 982-6000 or [email protected]. If you choose to write us rather than call us, please include the 7-digit reference number from your financial aid offer letter.
Appeal for Reconsideration Form -- (Completed by the Parent or the Student) This form should be submitted when making an appeal for reconsideration of special circumstances (e.g., loss of or reduced employment or excessive medical/dental expenses). Please contact our office by email at [email protected] to start a conversation about a possible appeal. Appeals for returning students will generally be considered after June 15, 2020. Both incoming and returning students should plan to submit their 2020-21 appeals by February 15, 2021, after which appeals will not be reviewed except in cases involving submission of a 2020 tax return, in which case you may be offered an extension.
Forms Requested by Student Financial Services
The forms linked below are typically completed only at the request of Student Financial Services, usually by means of a To Do List item being placed on the student's SIS account. Do not complete and submit any of the forms below unless specifically instructed to do so by Student Financial Services, as doing so will likely delay completion of your financial aid offer.
Asset Confirmation Request Form -- (Completed by the Parent or the Student) This form should be submitted when verifying Asset Information.
Child Support Paid Form -- (Competed by the Student and the Payer) This form is used to confirm the amount of child support paid by a parent or step-parent in 2018. For independent students, a parent may be the student or if married, the student’s spouse.
College Enrollment Verification Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the college enrollment of the student's family members.
Document of Support for Parent's Dependent Form -- (Completed by the Student AND the Parent) This form is used to confirm the household size of a dependent student’s parent(s).
Document of Support for Student's Dependent Form -- (Completed by an Independent Student) This form is used to confirm an independent student’s household size.
Family Income and Expense Form -- (Completed by the parent of a dependent student or by an independent student) Detailed list of monthly income and expenses for 2018.
Federal Benefits Received Form -- (Completed by the Student and the Parent) Generally, the Federal Benefits Received Form is requested because you reported that you or someone in your household received one or more of the following federal programs: Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP), Free or Reduced Price Lunch, Temporary Assistance for Needy Families (TANF), and Special Supplemental Nutrition Program for Women, Infants and Children (WIC), at any time during the 2018 or 2019 calendar year on the FAFSA.
Household Size Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the number of dependents within a household. Generally, the Household Size Form is requested when the number reported in the household size and/or number in college on the (FAFSA and the CSS/PROFILE. This form may also be requested in order to confirm the identity of the family members reported in the household size and/or number in college on an application.
Independent Document of Support Form -- (Completed by an Independent Student) This form is used to document the financial support given to additional members of your household (e.g. child).
Interest and Dividend Income Form -- (Completed by the Student and the Parent) This form is used to document interest and/or dividend income is reported on a federal tax return but for which no or very modest assets were reported that would explain the income generated.
